When you request records about you or someone else, Presbyterian Support Otago will ask you for proof of your identity.
For example, this could be your driver’s license or birth certificate.
To assist them in finding your records, they may ask you for:
Their Executive Administrator will search for the records you requested. If they need further information, they will contact you within one week of receiving your request.
If you are requesting records about a living whānau member, they will also ask you for written proof that the whānau member has given you permission to access their records.
If the whānau member cannot give you their permission but you are their legal representative under an Enduring Power of Attorney, Presbyterian Support Otago will ask you for proof of the Enduring Power of Attorney.
If you are requesting records about a deceased whānau member, they may also ask you for proof that you are the deceased whānau member’s legal representative.

Whether they will give you access to records about deceased whānau is decided by Presbyterian Support Otago on a case-by-case basis. If they will not give you access to the records they should tell you why.
