Even though Health New Zealand - Te Whatu Ora (Health NZ) is one organisation, each Health NZ location has its own process for requests. Use the process for the location you are sending your request to.
People who have made records requests to Health NZ have told us that the forms are complicated. They recommended that if you are filling one out by hand, they to print a few copies - or take your time filling it out.
When you have completed the form, check on the form how you can return it.
If you are requesting information from more than one Health NZ location, send the form to each location.
Under the Privacy Act you have the right to request your records in the way that works best for you and receive a response (for example, in writing or by phone). Health NZ can ask you to fill out a form, but they legally cannot require it.
Tip – However, completing their form will help your request get completed faster.
If you do not complete a form, find out what information to include in your request.
If you have any issues with your request, asking for your concerns to be raised as a complaint, or emailing the Health NZ privacy officer at hnzprivacy@health.govt.nz may solve the issue.
