If they cannot give you a copy of the records in the format you prefer, Health New Zealand - Te Whatu Ora (Health NZ) will let you know what options are available to you.
If you would like to pick up your records in person or view it in person, you can tell them in your request.
If you are in prison, you will only be able to receive a printed copy of the records.
