Under the Privacy Act, you have the right to request your records in the way that works for you best. This means you do not have to complete any forms if you do not want to. The organisation you are requesting records from must complete your request, no matter how you made your request.
But Health NZ will probably process your records request more quickly if you use their form.
You can use our Kōnae online records request form. Choose ‘Health NZ – Head office’ when selecting which record holder to send a request to. You can also request records from other organisations at the same time using our form.

You may get contacted by multiple regional Health NZ locations without warning and every Health NZ location may have different ways of completing to your request.
Tip: If you know which regional Health NZ location has your records, we recommend that you contact them. It will often be quicker then contacting the Health NZ head office.
